It is some years since I first learned about how risk was influenced by bureaucratic accountability in Diane Vaughan’s seminal book The Challenger Launch Decision. Understanding what goes on in an office is a key skill of leadership and business improvement. I was therefore intrigued by Lucy Kellaway’s new BBC Radio 4 series The Beginnings of the Modern Office. It proved to be well worth it.
